Duplicate comments in defect module QC 10

Hi,
I am having an issue in QC 10 where defect comments are duplicated for all users..Any help is really appreciated

For example: When a user clicks the 'Add Comment' button, they see something like:

---------------------------------------

Any ideas on how to resolve this ?
